PRE-QUALIFICATION OF SUPPLIERS

CHAK Guest House and Secretariat located on Musa Gitau Road off Waiyaki Way, Nairobi, are seeking to pre-qualify Suppliers for various goods and services f o r the year 2014/2015. Interested eligible firms may obtain further information and inspect the pre-qualification documents at CHAK Secretariat Offices.

CATEGORY B : SUPPLY OF SERVICES
B1	Provision of Taxi Services
B2	Booking of Air Travel, ticketing & related services
B3	Provision of Insurance brokerage services (Buildings, Office Equipment & Fittings, Motor Vehicles, Indemnity)
B4	Provision of localcourier services within Nairobi area and also outside Nairobi
B5	Provision of Motor Vehicle Repairs and Maintenance
B6	Provision of Office Cleaning Services
B7	Provision of Garbage Collection
B8	Provision of Sanitary Bins Services
B9	Provision of Refrigeration, Kitchen and Laundry Equipment Repair and Maintenance
B10	Provision of Accommodation and Conference Services outside Nairobi – in Kenya.
CATEGORY C: SUPPLY/INSTALLATION OF MEDICAL & GENERAL EQUIPMENT
C1	Supply and Installation of Theatre/ICU equipment, Service kits, Supplies & Accessories
C2	Supply and Installation of Imaging Equipment & Accessories
C3	Supply and Installation of General medical Equipment & Accessories

Requirements

i)       Certificate of Incorporation/Partnership deed for companies

ii)       Trading Certificate where applicable

iii)      PIN and VAT Certificates

iv)      Certificate from relevant authorities where applicable

v)       Tax Compliance certificate

vi)      Audited Accounts for the last three years of trading

vii)      A statement of cash flow for the most recent year of trading.

viii)      If no audited accounts available, a Bank letter outlining the current cash and credit position.

Pre-qualification documents can be obtained at a non-refundable fee of Ksh 2,000 per category from CHAK offices (Guest House Reception) during o f f i c e working hours. The Pre-Qualification documents shall be delivered to CHAK Secretariat Office enclosed in a plain sealed envelope clearly marked “PREQUALIFICATION OF SUPPLIERS 2014/2015” indicating the reference code no. and item description and should be addressed to;

All Prequalification documents should be received on or before 12 noon on Friday, August 29 2014. The submitted prequalification documents will be opened at 2:00pm same day at CHAK Boardroom and representatives of the applying firms are welcome to witness the opening.

Note. This is an application for prequalification and not tender for supply of goods/services. Only shortlisted firms will be requested to provide competitive quotations for the purchase of goods/services as and when needed. CHAK reserves the right to accept or reject any application and is not bound to give reasons for its decision.
